Fukiage Roten-no-Yu is a lovely little collection of shallow yet very hot pools at the end of a short trail into the woods. There are no changing facilities and it's mixed-gender, and can be busy at times. Bring any bath items you need, and plan on hot, mineral-rich water that will make your skin feel smooth and almost slimy. If you're lucky enough to get the place all to yourself it's truly as perfect an onsen as can be.
Getting there requires driving up a winding road towards the summit of Mt. Tokachi, then turning left just before you reach Tokachi Onsen (also worth a visit!). You park, then walk a few minutes to the pools.
